The Queensland Youth Alliance
Awards For Excellence
What
are the QYA Awards for Excellence?
The Queensland Youth Alliance Awards for Excellence recognise
outstanding achievements within the member organisations of the
Queensland Youth Alliance.
The annual awards program enables individuals aged between 16 – 28
years to be recognised as making an outstanding commitment to young
people.
The Awards are
offered in the following categories:
Leadership
Excellence
For demonstrating leadership and giving guidance to
young people and peers.
Inspiration
For those who inspire others through their actions or by
overcoming difficulties
Community
Service
For taking an active and responsible role in bettering
the community.
Who
is Eligable?
Nominees must be between the ages of sixteen (16) and twenty
eight
(28) and be a financial member of a QYA member organisation.
Please note that
self nominations will not be accepted.
Member
organisations must check that the nominee is a
current financial member of the QYA member organisation when the
application is received.

Organisations Need To Do?
WHEN
YOU SENT OUT THE NOMINATION FORMS
Member Organisations
will need to ensure that their members are aware of who to return
the nominations to.
WHEN YOU RECEIVE THE
NOMINATION
Member
Organisations
should check the following when they receive a nomination form:
q
Is the nomination form on the prescribed format?
q
Are up to 4 pieces of supporting documentation?
q
Is the nomination signed by the nominator, nominee and referees?
q
Are there 2 referees?
q
Have all the personal details of the nominator, nominee and referees
have been completed?
q
Has each of the selection criteria been completed?
q
Has the nomination been received by
5pm
on
31
May
2006?

ensure all information is treated as confidential.
WHEN
THE NOMINATIONS ARE ASSESSED
You will need to select a judging
committee within your member organisation to review the applications
select 1 nominee per category where appropriate.
You may determine the number of members on your judging committee
depending on your organisations needs.
The judging committee within your QYA Member organisation will
consider all nominations based on the selection criteria.
Nominations are to be assessed on the nomination only as presented
to the judging committee. QYA recommends that the assessment of he
application is by no other means than through the application
provided.
You have been provided with an evaluation grid, which will make the
process of short-listing consistent across all member organisations.
The evaluation grid includes:
• a line to indicate the award category
• a location for the surname of the applicant (you can evaluate up
to three applicants per form)
• the six assessment categories on the left hand side of the
evaluation grid
• a rating column with the predetermined ratings for each of the
categories
• a score column where the final score against the category for the
applicant will be inserted – the score is to be no higher than the
allocated rating. For example if the rating for the brief
description of the activity is 5, the score can be between 1 - 5
• comments against each criteria for each candidate to justify the
score
• the total score for each applicant
• provision for the signature of the assessing committee member
All categories will be assessed against the following criteria:
1. Brief description of the action undertaken
2. Amount of time dedicated to the action
3. Comprehensive explanation of the action
4. Explanation of why this action is a significant achievement
5. The results of the action being performed
6. Benefits of the action to the organisation and the community
You may commence your judging after nominations close at 5pm on
31 July 2010
ONCE YOU HAVE FINISHED JUDGING
Once you have determined your final
nomination in each category you will need to send them to the QYA
Chair at
Queensland Youth Alliance Awards for Excellence

All nominations to be considered by QYA must be received by the QYA
by 31 August 2010.
We regret that the QYA cannot accept nominations after the deadline.
QYA recommends you acknowledge the receipt of all nominations and
advise the unsuccessful nominees.
All decisions and recommendation of QYA are binding and final and no
correspondence will be entered into
Successful nominees will be advised in writing by the QYA of their
success in receiving an award.
QYA member Organisations will be advised of the outcome of the
judging.
Nomination
Deadlines
The
nomination forms MUST received by Member Organisation by 5.00
pm 31 July 2010.
Member Organisations must submit their nominations to the QYA by
31 August 2010.
